Impact and Programs
Accomplishments
Community Resource Connections (CRC) staff served 1,758 unduplicated individuals in the north central counties of Beltrami, Cass, Clearwater and Hubbard counties through 2,592 contacts and 3,212 referrals to 254 unique programs. Staff assisted 1,686 unique consumers in MNsure health insurance programming. Six consumers were successfully assisted in gaining access to Social Security Disability income. 24 Seniors were provided with Telephone Reassurance calls totaling 705. Community Resource Connections staff hosted and facilitated 12 Networking meetings with 74 unique agencies in Beltrami County and started Networking meeting in Clearwater County. We opened a new office in Hubbard County. We updated an online resource directory that serves the 4-county area and provided hard copy resource directories to agencies to use when they are working out in the field. CRC membership has risen to an all-time high of 58 agencies.
Current Goals
1) Maintain multiple office locations in each of the four counties we serve. 2) Maintain MNsure navigation services for the 4 county area. 3) Increase outreach and education concerning insurance options, Waiver services, and Senior Resources. 4) Assist outlying communities with access to adequate food resources. 5) Maintain networking activities in the 4 county area. 6) Increase membership in CRC by 10%.
